2025 Distinguished Alumni Award Nomination Form

The Illinois Community College Trustees Association's Distinguished Alumni Award recognizes the many success stories of Illinois' community college students.

Completed nomination forms and support materials are to be submitted no later than Thursday, Feb. 13, at 5 p.m.

Prior nominee names must be submitted each year for review and consideration. Family members and/or partners/spouses of the nominee may not submit an award nomination form. The award review committee will convene and evaluate all applications after Feb. 13.

Each nomination for the ICCTA Distinguished Alumnus Award will be reviewed according to the following criteria. Please complete the fields with extensive details or as much known.

1. Include how the nominee overcame life's obstacle(s).

2. Include how the nominee has outstanding success and distinction in their chosen field.

3. Include how the nominee's humanitarian service has proven greatly beneficial to society.

4. Include the nominee's continued interest in and support of education and community colleges.
