OPTIONAL: The Family Educational Rights and Privacy Act (FERPA) limits information that can be provided to individuals other than the student. This form allows the student to authorize individuals to receive information regarding their financial aid application and awards. A release can be provided for up to two academic years. Sandburg’s online service “mySandburg” allows the student to view information regarding awards, documents, billing etc. Students may create a Proxy through mySandburg to allow a designated person access to their account.
